SLMP Certificate Application
The SLMP Certificate Application is for students completing the School Library Media Program.
The following documents are needed in order to process your teacher certification application:
- Application filled out and signed by you
- If foreign born, evidence that you are legally present and eligible for employment
- Teacher Certification Release Form
- Authorization for certification (blue) form from Don Hamerly (to be completed during the exit interview.)
- Copy of your OTIS (Online Teacher Info System) account on isbe.net (Illinois State Board of Education) showing tests taken, date and passing statements
